Sports physicals must be updated EVERY YEAR for the next school year on or after April 15.  **The form below must be filled out completely and on file in the Athletic Office at BHS before participating with any athletic team.**

Parent or Guardian signatures are required in Sections 1, 4 and 5 - Student signatures are required in Section 3.
